Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
We accept checks as well as VISA, MasterCard, American Express, and Discover cards. We also give a 5% discount on balances greater than $500 when paid in cash. Payment is appreciated on the day of service.

Dental Insurance
We are part of the Delta Dental network as a Premier Provider. We can bill all other insurance plans; however, be advised that HMO, DPO, or PPO plans want you to see a dentist from their list of providers. Therefore they will pay minimally, if at all, for our services, and you are responsible for the remaining balance. Please be familiar with your specific dental insurance plan so we can partner together to make the most of your benefits.
